Using HealthAsyst
Our new system is a revolutionary change from past check-in processes. This is because it offers two ways to register for appointments: from the comfort of your own home or when you’ve arrived at one of our locations.
Those registering for appointments from home can virtually fill out and update all their information online. To register this way, you will need to:
- Click the registration link in the email or text that you received a few days before your appointment
- Confirm or update your information, such as demographics, insurance, and your clinical history
- Sign your consent forms digitally and safely from the comfort of your own home
How soon can I get an appointment?
Consultation appointments are typically made within two days, while surgery is usually scheduled within 1-2 weeks, sometimes sooner depending on the urgency and availability.
Do I have to have a referral to see a surgeon?
No, we have many elective surgeries that do not require a referral, but you should check with your insurance company.
About Your First Visit
Everyone at Evansville Surgical Associates is here to provide you the best service possible and to make the experience pleasant and convenient for you and your family.
Prior to your first visit, you will be asked to complete the appropriate forms via HealthAsyst. You will receive a text message or email a few days prior to your appointment with a link to our patient forms.
Please arrive 30 minutes prior to your appointment, and remember to bring the following:
- A Photo ID
- Your insurance card
- All prescription & non-prescription drugs you are currently taking, including any herbal supplements and vitamins. This is very important, as they can affect your surgery experience.
- Any X-rays or other imaging or lab work
Prior to your visit, we recommend that you call your employer’s benefits department or your insurer. It is your responsibility to know if you are required by your insurance plan to go to a specific hospital, lab, or x-ray facility. Please let our staff know if that is the case.
When you are called in for your appointment, you will be taken back to meet with the nursing assistant who will get your weight and ask you questions about what brought you to ESA and your symptoms before you are seen by the surgeon.
When the surgeon arrives, he or she will go over your symptoms, diagnoses, and discuss with you what your possible surgery options are, if any. The surgeons are there to answer any questions you may have and make you as comfortable as possible.
Depending on your insurance provider you may or may not owe a co-pay at the end of your appointment. Co-pays are due the day of your appointment. If a surgery is scheduled, your information will be given to a staff member who will contact your insurance company to determine what your coverage may be. Once again, this varies depending on what type of insurance you carry.
