What You Can Expect
Everyone at our office is at your service and works hard to make the experience as pleasant and convenient as possible — for you and your family.
Our staff will send you the appropriate forms to complete and bring with you to your appointment.
Please arrive 15 minutes prior to your appointment, and remember to bring the following:
- Completed Patient Data Form
- Completed Medical History Form
- Your Insurance Card
- All Prescription & Non-Prescription Drugs you are currently taking, including any herbal supplements and vitamins. This is very important, as they can affect your surgery experience.
Prior to your visit, we recommend calling your employer’s benefits department or your insurer. It is your responsibility to know if you are required by your insurance plan to go to a specific hospital, lab, or xray facility. Please let our staff know, if that should be the case. +
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Emergencies
An Evansville Surgical Associate is on-call for emergencies, 24 hours-a-day, 7 days-a week. Request an Evansville Surgical Associate surgeon in the emergency room, or call us at:812.424.8231
Insurance & Payment
We accept most plans. Coverage will be verified with your insurer at your first visit. Co-payment/co-insurance is expected at the time of visit. We accept cash, checks, VISA, and Mastercard. Photo ID is required.
